Health is everyone's responsibility.  We know that where we live matters to our health.  Our goal is to encourage all persons to eat healthy, stay active, and be well.  Through community prevention efforts and caring community partners, we can make Putnam County the healthiest county to live and work in.

Confidential services are provided to all, regardless of age, sex, income, race, disability, or marital status.

We accept and bill most private insurance, TennCare and Medicare.  Fees are based on family size and income.
