Please Note: All of the information submitted becomes part of the public record. The personal contact information is not required in order to submit a request. If you wish to remain anonymous, do not complete the contact information. However, if you would like to be contacted after we receive your request you will have to provide us with your return email address.

One of the things we need in order to investigate a complaint is the address of the violation. If the property does not have a street number visible, the County’s Mapping Services and Geographic Information Services (GIS) web page is an excellent resource for locating property addresses within Putnam County. It can be accessed at https://www.assessment.cot.tn.gov/RE_Assessment/SelectCounty.aspx. Having the street address (number and street name) helps us to take action on the service request quickly.
